* Verify that I am a candidate for weight loss surgery.

* Discuss weight loss surgery with my primary care physician.

* Call my insurance provider to determine whether weight loss surgery is a covered benefit for the treatment of morbid obesity.

* Verify that the medical provider I select is a participant on my insurance plan and, if applicable, is a network provider.

* Visit with my insurance provider regarding any additional information that will be required in order to qualify for weight loss surgery.

* Determine my portion of out-of-pocket cost or financial responsibility.
