Setting Goals:
Each individual admitted to the inpatient skilled nursing facility is evaluated and a care plan is developed, according to the patient's individual needs.  The team of the Physician, Nurses, Therapists, and Social Services meet weekly to assure the daily skilled need. Our mission is to help each patient attain the highest possible level of functioning.

Your Highly Skilled Treatment Team:
A Physician leads our specialized clinical team, which includes Physical Therapists, Occupational Therapists, Speech-Langauge Pathologists, Registered Nurses, Licensed Vocational Nurses, Social Services, and a dietitian.

The team interacts to design each individualized care plan and revise goals, if necessary. The patient is notified of upcoming discharge plans and goals. Because each patient's recovery process is different, each one is treated with highly
individualized care.
