Hillcrest Baptist Medical Center and Waco Cardiology Associates are again providing a wonderful service to our local teenage athletics this summer!

Physicians from Waco Cardiology Associates will conduct screenings for Hypertrophic Cardiomyopathy (HCM), the leading cause of sudden death in student athletes.  This is a FREE screening for student athletes, ages 15-18. (This screening does not replace a school sports physical.)

 

Parents need to be present to give authorization for this screening.  If a parent is not available, please print and sign the forms below for your child to participate.
