Finding out you are diabetic is life changing.

 

Adjustment can be hard and learning all you need to know to keep healthy can be a lot of work. In an effort to help people with diabetes, Hillcrest has established a Diabetes Self Management Education Program. The program provides education in one-on-one and group sessions to help those diagnosed with diabetes better manage their condition. A team approach is used to assist in the care of each client. Hillcrest team members include registered nurses, dietitians, and a certified diabetes educator who have the specialized training to truly make a difference.

 

Here’s how we’ll help…

 

The Hillcrest Diabetes Self Management Education program is designed to help empower you to take control of your diabetes.  You will learn what diabetes is, what causes it, and what you can do to prevent the long term complications such as eye, kidney, and nerve problems.  You will learn about the different treatment options that are available to help you control your diabetes.  You will learn how your diet, diabetes medicines, and your activity level all interact to influence your blood sugar levels, and what you can do in each of these areas to help achieve better control.  You’ll learn what to do if your sugars go too low, or if they go too high, and you’ll learn what to do if you become sick.  You’ll learn how to study your blood glucose readings and figure out what things may have caused the results.  You’ll learn how to set realistic goals to help you make positive lifestyle changes and improve your health.  Most importantly you will learn how YOU can control your diabetes, and not allow the diabetes to control you!

 

Here’s how to enroll…

 

Simply have your primary care doctor complete our DSME referral form (click here) and fax it to: 254-202-3297 (Attn: DSME).  We will then contact you to set up an appointment.  Most insurance plans help with the cost of diabetes education.
